You will need some personal information such as your name, address, social security number (SSN), date of birth (DOB), email address and phone number so that they can verify your identity with the SSN provided by the Internal Revenue Service (IRS) or other government agency where you have filed taxes during the last three years;
an image of your valid government ID;
proof of income if self employed;
bank account information if you want to use it as a debit card holder;
passport details if you are from outside the U.S.;
employment details if self employed; etc.,
but this depends on each case individually because there are many factors that determine whether someone is eligible or not for applying for this credit card or not.
